RadContour feature factsheet

A complete radiotherapy image-review and contouring workspace for iPad. Real DICOM in, real DICOM out — no cloud, no upload; everything runs on the device. This factsheet lists every tool and feature, organised by the workspace tab it lives in.

Research, education and training tool. Not a certified medical device. RadContour is not intended for diagnosis, treatment planning, treatment delivery or any patient-care decision. Every automatic result must be reviewed against the images by a qualified professional.

The workspace at a glance

RadContour opens to a patient library, then into a multi-planar workspace with six function tabs on the ribbon:

Any pane can show axial, sagittal, coronal or 3D; layouts are configurable.

Patient library & import

  • Patients auto-group from the DICOM Patient ID; expand to Study → Series / RT Structure Set / Registration / Plan / Dose.
  • Import from Files (a folder of DICOM), or query a DICOM server.
  • Per-patient edit, merge, delete; per-item swipe-to-delete.
  • Encrypted patient backup / restore (.rcbak), single or bulk.
  • Received-from-PACS studies arrive as notifications and open in place.

DICOM networking

Classic DIMSE, on a hospital-managed network:

  • C-ECHO (verification), C-FIND (Study-Root query), C-GET (retrieve — no C-MOVE needed), C-STORE (send, and receive as SCP).
  • Configurable local AE / port and a list of remote nodes (PACS / TPS).
  • Storage SCP to receive pushes, restricted to configured nodes (allowlist).
  • Uncompressed Implicit / Explicit VR Little Endian.

1 · Patient Data tab

  • Open for contouring — load an image set as the editable reference.
  • Load for fusion / registration — bring in a second image set.
  • Load as Ground Truth — a read-only reference set for Contour Assessment.
  • Structure sets — load any RT Structure Set of the patient onto the images.
  • Switch reference image set in place — contour on MR, copy to CT via a saved registration, without leaving the workspace.

2 · Contouring tab

Thirteen tools in all — twelve in the top grid, plus the AI auto-contour tile below (brush size shown only when a brush-type tool is active):

Auto-contour with AI

A dedicated tool tile:

  • Pick a model, then an organ preset (or "All organs"), then Run.
  • 25 TotalSegmentator organ models (CT + MR) plus the composite whole-body models, all on-device.
  • You pre-select organs via editable presets — never forced to run all 117.
  • Progress ring on the tile; results reviewed before use.
  • On a non-M iPad the tool explains it needs Apple silicon (a soft warning — the rest of the app still works).

Suggest contour (box-prompted)

LiteMedSAM / RepViT-MedSAM medical SAM models, or a classical heuristic, selectable in Settings.

ROI management

  • Unlimited ROIs, colour-coded, category-grouped (OAR / target / other).
  • Per-ROI visibility, colour, name, type; 3D surface preview.
  • Undo / redo; volume (cc) per ROI.

3 · ROI Geometry tab

Derive a new ROI from existing ones (RayPlan-style), each with a live preview and an editable, auto-suggested name:

  • Margin — uniform or anisotropic expand / contract.
  • Algebra — Boolean union / intersection / subtraction of ROIs.
  • Threshold — build an ROI from an HU window, with an enclosed-region fill.
  • Wall — a shell of a given inner / outer thickness.
  • External — automatic patient outline from the image, lungs included, with a two-slider live preview and a suggested setting.

4 · Registration tab

Sub-tabbed by method:

  • Auto — mutual-information rigid registration (Mattes MI, configurable metric / optimizer).
  • Manual — drag / rotate to align by hand.
  • ROI — structure-guided registration from matched ROI pairs.
  • Fusion display — Overlay, Checkers, Difference, adjustable blend.
  • Copy geometries — resample selected ROIs onto the registered set and save as a new RT Structure Set.
  • Import and export DICOM Spatial Registration (REG), including the inverse.

5 · Contour Assessment tab

Score an editable set against a loaded Ground Truth:

  • Dice similarity coefficient.
  • Hausdorff distance — maximum (mm).
  • Hausdorff distance — percentile (configurable, e.g. 95 %).
  • Mean (average symmetric) surface distance (mm).
  • Per-structure mapping; overlap fill; results table with a metric guide.

6 · Plan Evaluation tab

Load RT Dose / RT Plan on the reference CT:

  • Isodose wash (blue→red, normalised to the reference dose) and isodose lines, with editable levels (%, absolute cGy, colour). Drag the arrows on the colour legend to set the displayed dose range directly on the image.
  • DVH — cumulative dose-volume histogram, one curve per structure, with a scrub cursor (Ground-Truth solid / new dotted).
  • Dose statistics — Dmax / Dmean / D95 and dose-to-volume / volume-to-dose queries (%, cc).
  • Clinical goals — templated goal tables, pass / fail against the DVH.
  • Cross-image-set dose summation through a saved registration.
  • Deformed (NON_RIGID) dose is detected and evaluation is safely disabled.

Viewing & display (all tabs)

  • Axial / sagittal / coronal / 3D panes, configurable layout, maximise a pane.
  • Window / level — 12 built-in presets (Bone, Brain, Dental, Inner ear, Larynx, Liver, Lung, Mediastinum, Pelvis, Soft tissue, Spine, Vertebrae) plus custom presets.
  • Zoom / pan, orientation markers, info & zoom chips.
  • Contour line width, fill opacity, ROI volumes toggle — all in Settings.
  • Apple Pencil hover previews the brush before it touches down.

Export

  • RT Structure Set (strict, ascending-tag-order, RDP-simplified for DCMTK / Orthanc compatibility), default name "RadContour RTS".
  • Spatial Registration (REG), forward and inverse.
  • Anonymise on export (Patient Name / ID → ANONYMOUS; UIDs retained).
  • Send to a configured node via C-STORE, or export to a Files folder.

Accounts, security & privacy

  • Login at app open — three roles: Administrator, Medical Physicist, Radiation Oncologist, with a fixed capability split.
  • PBKDF2-SHA256 credentials in the Keychain; failed-login throttling; one-time recovery code.
  • Idle auto-lock (default 15 min), lock-on-background, optional Face ID — a UI lock that never discards unsaved contours.
  • App-switcher privacy shield — no patient data in the multitasking snapshot.
  • Encrypted at rest — patient store in the private container; encrypted, PHI-free diagnostic log with per-user attribution.
  • No analytics, no third-party SDKs, no telemetry. Patient imaging is processed on device and never sent to the developer.

Access & subscription

  • Free — up to 10 cases imported from Files, review in axial, sagittal, coronal, 3D and cine, and six hand tools (Navigate, Freehand, Polygon, Spline, Brush 2D, Erase 2D). Restoring a backup is always free.
  • RadContour Pro — adds the advanced brushes, AI auto-contouring, ROI geometry, registration and fusion, contour assessment, plan evaluation, DICOM networking, export, structure templates, backup and unlimited cases.
  • 7-day free trial on every Pro plan, then Monthly (€19.99) or Yearly (€149.99).
  • Monthly with a 12-month commitment (€14.99) — the annual price paid monthly (iPadOS 26.4+).
  • Access codes — device-locked, owner-issued codes grant a set number of days (used for evaluation programmes).

Requirements

  • iPad with an Apple M-series chip (M1 or later) — the on-device AI, 3D rendering and dose display need it.
  • iPadOS 26 or later.
  • Apple Pencil strongly recommended for contouring.

Technical documentation

The DICOM Conformance Statement documents exactly which services, SOP classes, transfer syntaxes and attributes RadContour supports — including its known limitations and deliberate deviations. It is written for imaging network architects and integrators connecting RadContour to a PACS or treatment planning system.

DICOM adherence does not by itself guarantee interoperability. This document supports a first-level comparison; it does not replace validation testing against the specific systems you intend to connect.
